Directions:  1.)  Review the official Festival Parade and Float Contest Rules, also available at www.ctownbeautification.com/parade.htm; 2.)  Complete this official registration form and mail it to:  Founder’s Day Parade Entry, c/o Parade Chairman, 304 Main Cross Street, Charlestown, IN 47111, before Saturday, June 23rd, 2007.  Hand delivery is accepted until 5 pm on Monday, June 25^th, 2007, at Charlestown City Hall, located at 304 Main Cross Street, Charlestown, IN, Attention:  Parade Chairman.  Completion and submission of this form registers the individual or group named below to participate as an entry in the 2007 Charlestown Founder’s Day Celebration Festival Parade, upon the approval of the Parade Chairman.
